We can currently connect patient data from over 60% of the U.S. health system patient portals. A PHR Plus user can sync health and fitness data from wearables and home medical devices into their PHR Plus account. Our platform is currently compatible with over 200 home medical and fitness devices.   Help me with care coordination: We use PHR Plus to improve care coordination. The most important part of the platform is that we allow users to share medical information with whomever they choose. Patients can electronically share their information from PHR Plus with doctors or loved ones for shared account access, or one-time view. A family member can ensure a loved one's PHR Plus profile is accurate from anywhere in the world, to help aging parents be empowered. By using the PHR Plus Smart Card a user can keep their medical information portable and accessible to reduce the risk for medical errors.
