HealthCentrix offers a patient-centric, cloud-based platform (Prevvy) to provide secure and seamless access to health data from different sources: primary care providers (PCPs), specialists, labs, hospitals, and patient-generated data. Our first product offers a unique solution for communication between patients, caregivers and PCPs for outside the clinic care, specifically, chronic care management, remote patient monitoring, and behavioral care management, services that are newly reimbursed by Medicare. We offer this platform free to patients and via a revenue-sharing model with primary care physicians.
