City Health Works is a non-profit business that rapidly trains peer coaches from low-income communities to educate and support families in their neighborhoods to better prevent and manage obesity-related chronic conditions. It empowers individuals and families to live healthier lives, reduces healthcare spending for health systems, and increases access to healthy businesses in low-income neighborhoods through financial incentives. They provide support for those struggling with obesity-related diseases (such as diabetes, heart disease, and hypertension) in the East Harlem community.
