CENTRAL QUEENSLAND, WIDE BAY, SUNSHINE COAST PHN

Central Queensland, Wide Bay, Sunshine Coast PHN is an independent not-for-profit organization funded by the Commonwealth Department of Health to improve the health outcomes of the region. PHNs coordinate primary and preventive care, i.e. care provided by practitioners outside hospitals, such as general practitioners, allied health professionals, chronic disease management, aged care, mental health, and indigenous health. They identify areas of need, such as lack of health care services, difficulty accessing these services, or regions with high health needs, and work closely with GPs, other health care providers, hospitals and other organizations to ensure patients are given the right care in the right place at the right time.
