Brisbane North PHN the goal is to create a community where everyone can live in perfect health. The PHN is committed to listening to the community and supporting the voice of consumers and career to develop and deliver the health and community care system for the people of the region. They strive to improve people and communities capacity to direct and impact system design. They are aware of the need to protect first nations peoples right to self determination. Rebalancing funding for community based services, such as in home care, is important to bring care forward to the patient place of residence. Consumers also need more power and information to manage their own care.
